HOME HEALTH: Medicare Certified Home Health Agency providing medically necessary clinical services pursuant to Medicare A guidelines for home health care. The services provided include skilled nursing, physical therapy, occupational therapy, speech therapy, medical social worker, home health aides and dietitians. The type of care provided is developed specifically for the individual patient (which is based on the attending physician orders). All care is provided in the patients home. HOSPICE: Medicare Certified Hospice Program providing services at the end stages of a patients life. All services are pursuant to Medicare A guidelines for Hospice care. Those services include skilled nursing, medical social worker, spiritual care, home health aides and others as needed. Volunteers play a significant role in Hospice and are mandated under conditions of participation. The focus of the care is pain and symptom management at the end stage of a patients life. The goal is to give the patient the best quality of life at this point in the disease process. Other than skilled medical care, the patient is provided such activities as music, pet therapy or in the case of patients with limited resources, donated funds can be used to provide transportation or meals.
