THE MISSION OF SAN YSIDRO HEALTH IS TO IMPROVE THE HEALTH AND WELL BEING OF OUR COMMUNITY'S TRADITIONALLY UNDER SERVED AND CULTURALLY DIVERSE PEOPLE. SAN YSIDRO HEALTH AND ITS PROGRAM SITES ARE DESIGNED UNDER THE CONCEPT OF THE PATIENT CENTERED MEDICAL HOME (PCMH) MODEL OF CARE, WHERE THE PATIENT, NOT THE SUPPORT STAFF OR PROVIDER, IS THE FOCUS OF ALL CARE AND SERVICES. PATIENTS HAVE ACCESS TO COMPREHENSIVE MEDICAL, DENTAL AND BEHAVIORAL HEALTH SERVICES AS WELL AS SUPPORT SERVICES AND PROGRAMS THAT HELP WITH OTHER NEEDS LIKE FOOD, HOUSING AND HEALTH EDUCATION.
