Agrace HME, LLC is a disregarded entity that was established to provide home medical equipment to Agrace hospice patients. With a fleet of delivery vans and a caring crew of DME technicians, Agrace delivers home medical equipment to our hospice patients. Agrace HME has two equipment warehouses, in Waunakee and Milton, where we use state-of-the-art software and systems to ensure medical equipment is delivered to patients quickly and accurately. Agrace Palliative Care, LLC is a disregarded entity that was established to provide in-home, specialized medical consultations for people who are living with any serious, life-altering illness. The service aims to improve the patient's quality of life by finding ways to better manage symptoms and stress related to the illness. Palliative care can be beneficial at any point in the disease including while curative treatment continues. Agrace Palliative Care providers work with the patient and their family to explore the challenges the patient and their doctors/health care providers have identified. These issues may be physical or psychosocial. Following a detailed consultation, Agrace generates recommendations for relieving the burden of illness for the patient and their providers to consider and implement. The service also assists the patient with planning for future health care goals through education, referrals to resources and matching treatment options with the patient's goals. Services are provided in the client's home to support independence: personal care, housekeeping, cooking, errands, companionship, exercise support, medication reminders and much more.
